Please note that practitioners must have a valid HIPAA compliant email address to receive the consultation note from our team. If you do not have a HIPAA compliant email at your office, you may provide a fax number to receive the consultation note.

You will also be required to provide a valid email address for your patient. Once the referral is complete, your patient will receive an email from us with a link to our online clinic portal where they will self-book their virtual appointment. Shortly after the successful completion of the consultation, you will receive a comprehensive report and plan to the secure email address or fax number that you have provided on our form.
